Health Insurance Invoice Errors!

Insurance carriers operate retroactively as I wrote about HERE

But for invoices and getting payment, they are the exact opposite and generate next month’s invoice about 25–28 days in advance — so the June invoice is produced in early May, the July invoice in early June, and so on.

Any changes submitted during the month for the next month, after the invoice is generated — terminations, new enrollments, dependent changes — haven’t synced into the carrier’s billing system yet when that invoice is produced.

This means a termination submitted in early May will almost certainly still appear on the invoice for the next month. That’s not an error — the invoice was already baked before the change hit the system. The correction will show up as a retroactive credit on the following month’s invoice, once the carrier has processed the termination.

This is the normal rhythm of group health insurance billing. Invoices always run behind the actual state of enrollment, and retroactive adjustments on subsequent invoices are how the system self-corrects. The thing to actually watch for: if a terminated (or enrolled) employee still appears as actively enrolled two invoice cycles later, that’s worth flagging — it means the termination may not have processed at all.
